How do I qualify for services?

Recovering from surgery? Facing a new or challenging diagnosis? Trouble understanding your medications? Have you had a recent fall? Home Care may be able to help. Home Health Care helps you get better, regain your independence, and become as self-sufficient as possible.

Medicare has established guidelines to help patients determine if they qualify for home care services. Medicare's coverage guidelines can be helpful to determine if you are eligible for home care services. In general, a patient who needs home care services would, under Medicare guidelines:

  • Have a difficult time leaving the house, typically with a taxing effort
  • Under the care of a physician
  • Receiving services under a plan of care prescribed by their physician and periodically reviewed for progress
  • In need of skilled nursing care on an intermittent basis, physical therapy or speech-language pathology, or has a continuing need for occupational therapy

Home Care might be right for you:

  • Recent exacerbation of a previous diagnosis
  • New medication or medication dosage change
  • Recent hospitalization
  • Having trouble living independently at home
  • Concerned about living alone
  • Have recently returned from a hospital stay or need IV fluids at home
  • Have a wound that's not healing
  • Have a chronic illness
  • Have trouble breathing
  • Have had recent hip or knee surgery and need help with mobility
  • Have fallen at home in the past few months
  • Experience chronic pain

Qualifying for Hospice Coverage

Hospice services can help provide comfort, dignity, and support to patients dealing with a life-limiting diagnosis. Patients of any age can qualify for hospice services.

How do I receive Hospice care?

  • The patient enrolls in an approved Hospice Program
  • The patient's physician uses his or her best clinical judgment to certify the patient is terminally ill with a life expectancy of six months or less, if the disease runs its normal course
  • The patient chooses to receive hospice care rather than curative treatments for the illness
  • Bee First Hospice can provide hospice services in the home, an assisted-living community, nursing center, or hospital.
