Comprehensive Services for a Higher Quality of Life
We are committed to your care and will fulfill this responsibility by providing the following:
Nursing Services – Skilled nursing services, such as catheter care, colostomy care and wound dressing, are provided by professional registered and/or licensed vocational nurses under the direction of the supervising nurse.
Home Health Aid – Our home health aides lend valued support to patients and their family by providing personal care services, including assistance with activities of daily living.
Medical Social Services – Licensed medical social workers provide services to help alleviate stresses encountered during illness, offer helpful information and support, and assist in long-term planning when needed.
Bereavement Services – Bereavement support is provided to family members for a period of time following the death of a loved one. Personal contact and written information is made available to help families through the grieving process.
Spiritual Counseling – A hospice chaplain is available to provide spiritual support during this difficult time. They offer counseling to patients and families or assist with referral to clergy in the community if desired.
Pharmacy Services – Prescription and non-prescription medicines are covered under hospice care unless otherwise arranged by the patient’s insurance company. Our hospice caregivers will explain medicines as well as assist with the scheduling of medicine intake.
Nutritional Counseling – Your dietary needs are important. Our hospice team assists you and your family to ensure all requirements are met. A consultation with a nutritionist is an option for those with particular nutritional needs.
Medical Equipment and Supplies – Gathering medical supplies and equipment can be stressful for family members dealing with the daily demands of care-giving. We arrange for supplies, medical equipment and pharmaceuticals as needed so you can spend more time with your loved one.
Inpatient Respite – Respite care provides breaks for caregivers at home by caring for hospice patients and providing symptom management.
Pain and Symptom Control – Our hospice team develops a plan that helps relieve pain and other uncomfortable symptoms patients may experience as the result of an illness.
Volunteer Support – Trained hospice volunteers provide brief caregiver respite and companionship. They can also help with daily tasks such as running errands and preparing meals.
Physician-Directed Medical Services – The hospice medical director, under the supervision of your or your loved one’s primary physician, provides services and makes home visits as needed.
Homemaker Services – When a terminal illness prevents you from performing basic, household chores like housekeeping and meal preparation, our homemakers are available to ensure that you maintain your independence.
